Value Transformation has launched a new telehealth service to support safe, effective care at home for patients across Wales.
It uses a digital platform that enables clinicians to monitor patients, and guide care decisions remotely.
It is helping to expand clinical capacity, reduce pressure on acute services, and improve patient outcomes through proactive, digitally supported care.
The telehealth initiative is part of a wider set of technology-enabled virtual care services. It focuses on improving care using digital tools, without losing sight of the need for strong clinical leadership. All services are led by local NHS Wales teams and fully integrated into existing care pathways.

The rollout is fully supported by the Value Transformation Team, as is the technology throughout the design, mobilisation and evaluating phases.
Our telehealth service supports NHS Wales Performance and Improvement's commitment to care transformation using virtual care and fully aligns with aims of A Healthier Wales.
It offers a clinically appropriate alternative to hospital admissions, helps prevent deterioration, and can support earlier discharge by maintaining active oversight at home.
Clinicians can check patients' health using a combination of patient-reported symptoms and device readings. This enables prompt intervention and supports continuity of care in a community setting.
