How telehealth supports patient care

The telehealth platform offers a real-time view of patient information. It allows clinical teams to oversee care remotely while keeping high standards of safety and responsiveness.

The platform enables clinical staff to:

Monitor health data in real time

  • Vital signs (e.g. heart rate, oxygen saturation, temperature)
  • Patient-reported symptoms via digital forms
  • Instant upload to clinician dashboard

Prioritise care based on clinical need

  • Visual indicators (e.g. Red/Amber/Green) highlight patients needing attention.
  • Enables efficient caseload management.

Receive prompt alerts

  • Automated alerts when readings fall outside clinical thresholds.
  • Supports early intervention and a suitable clinical response.

Communicate with patients securely

  • Clinicians can message patients or set up video calls.
  • Supports reassurance, escalation, or clarification without a home visit.

All clinical decision-making and patient contact is managed locally by clinical teams.

The platform enables a proactive approach to care, allowing earlier identification of deterioration and helping to avoid escalation where possible.

Implementation across Health Boards

The telehealth service is live across multiple health boards in Wales.

Initial focus areas include Acute Medicine, Respiratory, Cardiology and Frailty.

However, the approach is designed to be flexible and scalable, with further development planned across a wide range of clinical pathways such as:

  • Surgical recovery
  • Oncology
  • Palliative and end-of-life care
  • Paediatrics
  • Long-term condition management

Cwm Taf Morgannwg UHB was one of the first Health Boards in Wales to adopt the telehealth model, supporting patients on the Acute Medicine pathway.

The Directorate Manager for Urgent Primary Care said: “I don’t think any of us were expecting the ‘go-live’ to go quite as well as it did with three patients on the first day. To find ourselves agreeing to doubling the virtual beds after just one week is incredible.

This is excellent news and a testament to all the hard work that has gone into getting this project off the ground.”

Patients have also provided positive feedback.  A senior nurse in urgent care shared with us one of their patients' response: “She was very pleased to be using the service. She found the equipment very easy to use and felt reassured knowing a doctor would review her observations during the day.”

How can Health Boards and Trusts be involved?

The service is available to all health boards in Wales and is centrally supported by the Value Transformation team within NHS Wales Performance and Improvement.

Support includes:

  • Clinical and operational planning
  • Assurance and governance
  • Staff training and onboarding
  • Equipment and logistics
  • Evaluation and reporting
