Transitional care

Transition is more than just the move from paediatric to adult diabetes care: it’s a critical process that ensures young people feel supported, prepared, and empowered at every stage.

Effective transitional care goes beyond clinical management; it requires a coordinated approach that considers emotional well-being, self-management skills, and the challenges young people may face as they take increasing responsibility for themselves, and their diabetes. 

The goal is to equip young people with the confidence and skills they need to stand on their own two feet by the time they reach 25, the age at which brain development is largely complete, decision-making abilities are fully matured, and young people are expected to manage their health and responsibilities independently.

Our services must reflect this understanding, ensuring they are flexible, responsive, and tailored to meet the evolving needs of young people throughout this journey.
