A new interactive digital document highlighting the key achievements and experiences of cluster working in Wales is available now.
‘Cluster Working in Wales’ has been put together by NHS Wales Performance and Improvement’s Strategic Programme for Primary Care team.
Its release is part of the refreshed narrative of the Primary Care Model for Wales (launched in 2018), recently announced by Judith Paget, Chief Executive, NHS Wales and Welsh Government’s Director General for Health, Social Care and Early Years.
While ‘Cluster Working in Wales’ is a comprehensive document, its interactive nature means users will be able to go direct to the content and resources they want.
It is full of information on who the individual clusters are, the make-up of the multi-disciplinary teams, the geographical area they cover, along with their key achievements, what work has been completed and what can be learned from that and what is being worked on right now. It also includes the Pan Cluster Planning Groups in the same format.

Sue Morgan is the National Director of the Strategic Programme for Primary Care in NHS Wales Performance and Improvement and said: “It is fantastic to see outstanding examples of cluster working in Wales all in one place and shows the contribution of the Primary Care Model for Wales as set out in A Healthier Wales.
“This is a comprehensive document, but as it is interactive you’ll be able to dip in and out, keep coming back to it and reuse it as a valuable resource.”
Sue added: “It will also help with learning from others’ experiences. Both the clusters and the Pan Cluster planning groups highlight their key achievements, but more importantly, what worked well for them and what did not work quite as well. So, asking - what would they have done differently? This means we can all learn from each other in the cluster space, avoiding duplication and keep the focus firmly on improving patient outcomes.”
As well as the Wales-wide document, each health board will receive its own specific version tailored to its area. There is also a series of videos available highlighting the work that has been done in the Cwmtawe cluster, part of Swansea Bay University Health Board that can be accessed here.
