This period can be particularly challenging for individuals with diabetes, as they often encounter difficulties discussing certain topics during clinical visits and may lack age-appropriate resources and support.
The initiative to create SEREN Connect began with discussions within the diabetes community, where it became clear that many individuals shared a similar narrative of struggling to discuss important issues during clinic visits. Healthcare professionals working in diabetes services also identified several key challenges, including a lack of resources and time to address the unique needs of young people during this transition period. Additionally, the logistics of coordinating meetings with multiple individuals in one space were seen as cumbersome and potentially awkward for both patients and healthcare providers.
In response to these challenges, Sara Crowley, Diabetes Transitional Care Coordinator for the All Wales Diabetes Implementation Group, took the initiative to explore solutions. Collaborating with the team behind SEREN Diabetes at Diagnosis, a paediatric diabetes education program in Wales, she formed a sub-group focused on developing a program specifically tailored to addressing the disconnect experienced by young people during the transition from paediatric to adult diabetes care in the UK.
In 2019, SEREN Connect was officially launched. This innovative program aims to provide age-appropriate resources, support, and a safe space for young individuals with diabetes to discuss their concerns and needs as they navigate this critical period in their healthcare journey.
SEREN Connect goes beyond traditional healthcare services by addressing the distinct needs of young adults with diabetes during this transition, empowering them to manage their condition effectively while engaging in various aspects of young adult life, such as driving, drinking alcohol, socialising, disclosing their condition to new acquaintances, navigating the NHS, pursuing employment opportunities, and traveling.
This comprehensive approach incorporates the expertise of healthcare professionals who facilitate the program, as well as the invaluable dimension of peer support. Many young people with diabetes may find this transitional phase isolating, and SEREN Connect aims to provide a supportive community where participants can openly discuss their concerns and experiences.
SEREN Connect's creation reflects a proactive effort to improve the transitional care experience and bridge the gap between paediatric and adult diabetes services specifically tailored for the benefit of young patients in the United Kingdom.
