Advance and Future Care Planning - Video resources for the public

None of us wants to think of ourselves as so ill that we couldn’t decide for ourselves, but this does happen to many people, and if you haven’t made your own wishes known then someone else will be deciding for you.

This site draws on the expertise of NHS Wales, the National Council of Palliative Care, Hospice UK as well as other organisations from across the country to help you start thinking about your Advance Care Plan.

Bereavement

Information about charities and trusts that offer advice and support, and links to bereavement services in each local health board.

Talk CPR

Talk CPR’s goal is to encourage conversation about CardioPulmonary Resuscitation (CPR) for people affected by life-limiting and palliative illnesses. Talking about Do Not Attempt CardioPulmonary Resuscitation (DNACPR) is an important part of advance care planning and can help minimise distress at a later stage.

DNACPR

'Sharing and Involving - a clinical policy for Do Not Attempt Cardiopulmonary Resuscitation (DNACPR) for adults in Wales', a summary of the key changes in version 4 (2020), and information for patients.

Compassionate Cymru

Compassionate Cymru works with individuals, organisations and communities to improve how people in Wales care, die and grieve.

Hospice UK

We work closely with Hospice UK and with the individual third sector and independent services that support and provide end of life care services throughout Wales.

What Matters Most Charter

We support the 'What matters most' community approach to advance care and treatment planning and the associated charter.
