Everyday diabetes management is about finding routines that help keep your blood glucose as stable as possible. Your healthcare team will use information like your blood test to understand how your treatment is working, but most of the day‑to‑day management happens with you, and these pages offer trusted links to help you learn more.

For people with Type 1 diabetes, support is offered at different life stages. Children and young people are usually offered SEREN education; adults may be offered DAFNE, and SEREN Connect provides resources aimed at under‑25s (but other age groups may still find this helpful), including easy‑read formats, to support the move to adult services and navigate young adult life.

For people with Type 2 diabetes, you can sign yourself up to the following:

  • MyDESMOND is a programme for people with type 2 diabetes to support their self-management.
  • Diabetes UK Learning Zone offer online learning and tools to support everyday self‑management.
  • Pocket Medic provide a series of films that can help you understand more about your diabetes.

Adults with Type 2 Diabetes will be offered Diabetes support groups such as Diabetes Awareness Sessions, XPERT Diabetes, XPERT Insulin and Diabetes self-management programmes. Speak to your healthcare provider about how you can access these groups.
