Type 2 Point of Diagnosis toolkit

A new diagnosis of type 2 diabetes can feel overwhelming. The Point of Diagnosis Packs have been developed for this very reason - to support people and healthcare professionals at this important moment, helping turn diagnosis into clear, practical next steps.

The packs include:

  • For people newly diagnosed with type 2 diabetes

A patient guide, Living Well With Type 2 Diabetes, covering what usually happens after diagnosis, what to do first, common questions, remission, essential health checks, medicines, food, movement, blood glucose monitoring, emotional wellbeing and trusted support routes.

  • For healthcare professionals

A clinician-facing Point of Diagnosis Pack to support clear, safe and person-centered diagnosis conversations in primary care, community services, outpatient clinics and hospital settings. It includes immediate priorities, safety-netting, communication guidance, essential care processes, structured education prompts and handover templates.

Developed with clinicians and people living with diabetes

These resources have been co-designed with people living with type 2 diabetes and multidisciplinary healthcare professionals ensuring the packs are clinically relevant, practical for routine care and grounded in the real experiences of people receiving a new diagnosis.

 

The content uses clear, supportive and non-judgemental language. It recognises that people may feel shocked, worried or unsure after diagnosis, while helping them focus on manageable actions that can improve confidence, self-management and long-term health.

Supporting behaviour change from day one

The packs are designed to help people move from information to action. They encourage small, achievable first steps such as booking essential checks, understanding medicines, accessing diabetes education, making one food change and choosing one realistic movement goal. This approach helps build confidence and avoids overwhelming people at diagnosis.

 

For clinicians, the pack supports consistent conversations that are clear, safe and person-centered - including how to explain the diagnosis, check understanding, record access needs and make the next step explicit.
